Using mindfulness-based therapy with women suffering from sexual dysfunction

Your patient says her body is having sex while her mind is somewhere else entirely. Dr. Lori Brotto (University of British Columbia), a world-leading clinician-researcher with over 20 years developing mindfulness-based treatments for female sexual dysfunction, shows step by step how to bring patients back into their bodies – whether the issue is low desire or genital pain.

About the Training

Mindfulness isn't explained to the patient – it's something she discovers for herself, one raisin at a time

Two of the most common female sexual dysfunctions – low desire and genital pain – respond to the same core skill: present-moment, non-judgmental awareness

Dr. Brotto opens with the same exercise she uses with every new patient: a short eating meditation with a raisin. You'll learn why she never explains mindfulness before practicing it, and how a three-question "inquiry" after each exercise lets patients draw their own connection between the practice and their sexual difficulties – rather than being told how it works.

A clinician-researcher who has spent over 20 years developing and testing mindfulness-based treatments for sexual dysfunction shows, exercise by exercise, how to build an evidence-based program for low desire and genital pain.

Dr. Brotto begins with the eating meditation she uses in her own practice with every new patient, then teaches the three-question „inquiry” that follows every mindfulness exercise – what did you notice, how was this different from how you normally do it, and how is this relevant to your sexual difficulties. This structure, borrowed from mindfulness-based cognitive therapy for depression, lets patients discover the relevance of mindfulness for themselves rather than being told how it works.

The training walks through the full 8-session group program Dr. Brotto developed for women with low sexual desire – eating meditation, body scan, mindful movement, mindfulness of thoughts, a home „focusing” exercise done in the shower, a mirror exercise for genital self-awareness, and sensate focus. You’ll see the outcomes from her randomized controlled trial of 148 women: significantly greater reductions in sexual distress than a sex-education control group, improvements maintained a full year later, and the psychological mechanisms that explain why it works – greater self-compassion, better interoceptive awareness, less self-criticism, fewer depressive symptoms.

A separate block covers Dr. Brotto’s adaptation of this program for provoked vestibulodynia – genital pain triggered by touch, affecting roughly 16% of women. You’ll learn the „two arrows” story she uses to explain the difference between pain and suffering, the pain-elicitation exercise she runs in session, and the results of her randomized trial comparing mindfulness to cognitive behavioral therapy in 130 women with vestibulodynia: mindfulness matched CBT and, on self-reported pain, outperformed it, with large effects on sexual distress and pain catastrophizing maintained a year later.

The training closes with the concept of equanimity – bringing the same non-judgmental acceptance to every sensation, pleasant or not – illustrated with Rumi’s poem „The Guest House,” and the portable three-minute breathing space patients can use before a sexual encounter or any stressful moment.

Who It's For

Who is this training for?

Therapists working with low sexual desire

You have patients who say their body is present during sex but their mind is somewhere else, and you want a structured, evidence-based program to offer them beyond psychoeducation alone.

Therapists and gynecology-adjacent clinicians working with genital pain

You see patients with vulvodynia or provoked vestibulodynia whose pain has no visible cause on examination, and standard treatments like topical agents or gabapentin haven't helped.

Psychologists and sex therapists wanting evidence-based mindfulness tools

Whatever your therapeutic orientation, you want concrete, research-backed exercises for bringing present-moment awareness into sexual health work.

Helpful to have: basic familiarity with CBT or sex therapy concepts

This is the original English-language recording. No prior meditation or mindfulness practice of your own is required – Dr. Brotto teaches every exercise from the ground up, in a form you can use with patients from your very next session.

Outcomes

What You'll Learn

You'll understand...

  • the scope and prevalence of female sexual dysfunction, and which patients are the best candidates for a mindfulness-based approach
  • the psychological mechanisms that explain how mindfulness improves low sexual desire – greater self-compassion, interoceptive awareness, and fewer depressive symptoms
  • the mechanisms specific to genital pain – reduced pain catastrophizing and greater self-compassion – and how they differ from the mechanisms of low desire

You'll know how to...

  • run the three-question "inquiry" after a mindfulness exercise so patients draw their own connection to their sexual difficulties
  • structure an 8-session mindfulness program, from eating meditation through sensate focus, and adapt it to individual or shorter-format work
  • use mindfulness alongside a CBT framework – observing a thought as a mental event rather than challenging or restructuring it

You'll practice...

  • guiding a short eating meditation and a body scan, and leading the inquiry that follows
  • the "two arrows" exercise for separating physical sensation from the suffering added on top of it
  • normalizing common barriers patients raise – irritation, fatigue, impatience, shame around touch – without pushing back against them

Program

What the Training Covers

  1. Foundations of Mindfulness and the Inquiry

The history of mindfulness in Western medicine – Thich Nhat Hanh, Jon Kabat-Zinn's work with chronic pain patients, mindfulness-based cognitive therapy for depression – and the three-question inquiry that turns every practice into a patient's own discovery.

  2. An 8-Session Program for Low Sexual Desire

Eating meditation, body scan, mindful movement, mindfulness of thoughts, the home shower "focusing" exercise, the mirror exercise, and sensate focus – with outcomes from a 148-woman randomized controlled trial and the mechanisms that explain them.

  3. Sexual Sensations Awareness and Working with Aversion

Pairing fantasy, erotica or a vibrator with a guided awareness practice – including its use with cancer survivors who had lost genital sensation – and how to work with the aversion and attachment that come up around touch.

  4. Equanimity and Closing Practices

The concept of equanimity, Rumi's poem "The Guest House" as a teaching tool, and the three-minute breathing space patients can carry with them into any sexual encounter or stressful moment.

  5. Mindfulness for Genital Pain (Provoked Vestibulodynia)

The "two arrows" story distinguishing pain from suffering, the in-session pain-elicitation exercise, and results from a 130-woman trial comparing mindfulness to cognitive behavioral therapy for vulvar pain.
